Family Health is a Community Health Center in Greenville, Ohio located in Darke County (home of Annie Oakley). Our mission at Family Health is
to help our patients build healthy lives for themselves and their families. We believe

We strive to answer your call by the third ring. It doesn't always work out that way. If you are placed on hold, we will get back with you as soon as possible. If you call long distance or are unable to hold, please let us know. We will return your call as soon as possible.
Call 548-9680 to schedule an appointment. So that we can allow adequate time for you, it is important to fully describe the nature of your problem, even though it may be difficult and uncomfortable.
If you are unable to keep an appointment, 24 hours notice would be appreciated so we can be available to meet someone else's needs.
New Patients should arrive 20 minutes before appointment time to complete registration forms.
Acute Illnesses will be scheduled with your doctor. If he/she is not in, you can be scheduled with one of the other doctors, the nurse practitioner, or our physician's assistant.
Routine Exams should be scheduled at least 1 month in advance or before you leave the office.
Medication Refills should be called in at least one week before completion of your prescription. When you call, please give the patients name, date of birth, phone number, name of the medication, and the drug store.
Birth Control Pill Refills should be called in at least one month in advance.

We are open 8:00 am to 5:30 pm on Mondays. 8:00 am to 5:00 pm Tuesday through
Friday.
